SeniorCare Midwest, Inc. ("SeniorCare Midwest," "we," "us," or "our") respects your privacy. This policy explains our practices for the public website at seniorcaremidwest.com and the linked public consultation and caregiver-application forms.

This policy applies to information collected through our public website and public intake forms, as well as information you provide when contacting us about services or employment opportunities. It does not replace privacy or confidentiality terms contained in a client service agreement, employment documentation, or a separate secure portal notice.

Important: Our public forms are not monitored for emergencies and are not intended for medical records or highly sensitive information. Call 911 for an emergency.

  • Care-request information, such as the type of non-medical support requested, general timing, approximate schedule, service location, and a description of assistance needed.
  • Caregiver-applicant information, such as credentials, experience, availability, preferred assignments, transportation, and preferred service areas.
  • Communication information, including messages, questions, call notes, and your consent to be contacted about the request you submitted.
  • Submission records, including a reference number, the date and time of submission, the form used, and the website source that led to the form.

If you voluntarily include health-related circumstances in a message, we use that information only as reasonably necessary to respond, evaluate whether our non-medical services may be appropriate, and plan next steps. Please do not submit diagnoses, medical histories, medical documents, Social Security numbers, banking or payment details, insurance numbers, passwords, or identity documents through a public form.

Information collected automatically

Our website or hosting providers may receive limited technical information such as an IP address, browser or device type, requested pages, referring page, and date and time of access. We may also use short-lived browser session storage to preserve campaign parameters, such as the source of a website visit, while you move between pages. The public form uses security controls such as rate limiting and duplicate-submission protection.

Submitting a telephone number or email address allows us to respond to the request you initiated. It does not by itself enroll you in unrelated marketing or automated advertising messages.

  • Authorized personnel who need it to handle intake, scheduling, recruiting, compliance, or business operations;
  • Service providers that support website hosting, secure data storage, communications, security, or other business functions and are permitted to use information only for those services;

We do not sell personal information. We also do not share personal information with third parties for cross-context behavioral advertising.

Our current public website does not use third-party advertising cookies. It may use browser session storage to remember limited referral or campaign information during the active browsing session. Hosting and security providers may use essential technologies or logs needed to deliver and protect the website. You can clear browser storage through your browser settings, though some website functions may be affected.

Because we do not currently use personal information for targeted advertising, the website does not presently respond to browser "Do Not Track" signals. If our practices change, we will update this policy and provide any legally required choices.
